No matter your age, planning for future medical decisions is probably not at the top of your list. However, there may be a time when you are unable to speak for yourself. Who will carry out your wishes and make them known to your healthcare providers?
Whether you have stroke, a terminal illness, or an accident that leaves you unresponsive, your wishes can still be carried out. You decide and record your requests in an advanced care directive. It allows you to communicate the kind of care you want and the kind of care you do not want, leaving no questions about your care. This not only helps fulfill your desires but gives your family and friends a definite understanding of what type of medical care and/or spiritual support you want.
Choices Connection is a comprehensive advanced care planning service created in partnership with Generations, Genesis and Trinity Hospitals. Trained volunteers can take you through the process and get your wishes on paper. We have both Iowa and Illinois Durable Power of Attorney forms for health care directives. Your advance care planning document will be placed on file at either Genesis or Trinity Hospitals. This is a FREE service. Contact Generations to schedule a planning session.
